Cleveland, Ohio clinic performs US’s first face transplant

A team of eight transplant surgeons in Cleveland Clinic in Ohio, USA, led by reconstructive surgeon Dr. Maria Siemionow, age 58, have successfully performed the first almost total face transplant in the US, and the fourth globally, on a woman so horribly disfigured due to trauma, that cost her an eye. Two weeks ago Dr. Siemionow, in a 23-hour marathon surgery, replaced 80 percent of her face, by transplanting or grafting bone, nerve, blood vessels, muscles and skin harvested from a female donor’s cadaver.

The Clinic surgeons, in Wednesday’s news conference, described the details of the transplant but upon request, the team did not publish her name, age and cause of injury nor the donor’s identity. The patient’s family desired the reason for her transplant to remain confidential. The Los Angeles Times reported that the patient “had no upper jaw, nose, cheeks or lower eyelids and was unable to eat, talk, smile, smell or breathe on her own.” The clinic’s dermatology and plastic surgery chair, Francis Papay, described the nine hours phase of the procedure: “We transferred the skin, all the facial muscles in the upper face and mid-face, the upper lip, all of the nose, most of the sinuses around the nose, the upper jaw including the teeth, the facial nerve.” Thereafter, another team spent three hours sewing the woman’s blood vessels to that of the donor’s face to restore blood circulation, making the graft a success.

The New York Times reported that “three partial face transplants have been performed since 2005, two in France and one in China, all using facial tissue from a dead donor with permission from their families.” “Only the forehead, upper eyelids, lower lip, lower teeth and jaw are hers, the rest of her face comes from a cadaver; she could not eat on her own or breathe without a hole in her windpipe. About 77 square inches of tissue were transplanted from the donor,” it further described the details of the medical marvel. The patient, however, must take lifetime immunosuppressive drugs, also called antirejection drugs, which do not guarantee success. The transplant team said that in case of failure, it would replace the part with a skin graft taken from her own body.

Dr. Bohdan Pomahac, a Brigham and Women’s Hospital surgeon praised the recent medical development. “There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Leading bioethicist Arthur Caplan of the University of Pennsylvania withheld judgment on the Cleveland transplant amid grave concerns on the post-operation results. “The biggest ethical problem is dealing with failure — if your face rejects. It would be a living hell. If your face is falling off and you can’t eat and you can’t breathe and you’re suffering in a terrible manner that can’t be reversed, you need to put on the table assistance in dying. There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Dr Alex Clarke, of the Royal Free Hospital had praised the Clinic for its contribution to medicine. “It is a real step forward for people who have severe disfigurement and this operation has been done by a team who have really prepared and worked towards this for a number of years. These transplants have proven that the technical difficulties can be overcome and psychologically the patients are doing well. They have all have reacted positively and have begun to do things they were not able to before. All the things people thought were barriers to this kind of operations have been overcome,” she said.

The first partial face transplant surgery on a living human was performed on Isabelle Dinoire on November 27 2005, when she was 38, by Professor Bernard Devauchelle, assisted by Professor Jean-Michel Dubernard in Amiens, France. Her Labrador dog mauled her in May 2005. A triangle of face tissue including the nose and mouth was taken from a brain-dead female donor and grafted onto the patient. Scientists elsewhere have performed scalp and ear transplants. However, the claim is the first for a mouth and nose transplant. Experts say the mouth and nose are the most difficult parts of the face to transplant.

In 2004, the same Cleveland Clinic, became the first institution to approve this surgery and test it on cadavers. In October 2006, surgeon Peter Butler at London‘s Royal Free Hospital in the UK was given permission by the NHS ethics board to carry out a full face transplant. His team will select four adult patients (children cannot be selected due to concerns over consent), with operations being carried out at six month intervals. In March 2008, the treatment of 30-year-old neurofibromatosis victim Pascal Coler of France ended after having received what his doctors call the worlds first successful full face transplant.

Ethical concerns, psychological impact, problems relating to immunosuppression and consequences of technical failure have prevented teams from performing face transplant operations in the past, even though it has been technically possible to carry out such procedures for years.

Mr Iain Hutchison, of Barts and the London Hospital, warned of several problems with face transplants, such as blood vessels in the donated tissue clotting and immunosuppressants failing or increasing the patient’s risk of cancer. He also pointed out ethical issues with the fact that the procedure requires a “beating heart donor”. The transplant is carried out while the donor is brain dead, but still alive by use of a ventilator.

According to Stephen Wigmore, chair of British Transplantation Society’s ethics committee, it is unknown to what extent facial expressions will function in the long term. He said that it is not certain whether a patient could be left worse off in the case of a face transplant failing.

Mr Michael Earley, a member of the Royal College of Surgeon‘s facial transplantation working party, commented that if successful, the transplant would be “a major breakthrough in facial reconstruction” and “a major step forward for the facially disfigured.”

In Wednesday’s conference, Siemionow said “we know that there are so many patients there in their homes where they are hiding from society because they are afraid to walk to the grocery stores, they are afraid to go the the street.” “Our patient was called names and was humiliated. We very much hope that for this very special group of patients there is a hope that someday they will be able to go comfortably from their houses and enjoy the things we take for granted,” she added.

In response to the medical breakthrough, a British medical group led by Royal Free Hospital’s lead surgeon Dr Peter Butler, said they will finish the world’s first full face transplant within a year. “We hope to make an announcement about a full-face operation in the next 12 months. This latest operation shows how facial transplantation can help a particular group of the most severely facially injured people. These are people who would otherwise live a terrible twilight life, shut away from public gaze,” he said.

Putting on a seminar involves planning, time, and money. Understanding the ins and outs of the industry will go a long way in helping you hold a successful seminar. Thats where I come in. Over the past 4 years, I have put on over 4,000 seminars, and a handful of national conventions. I will help you plan the event, and make sure that someone shows up!

The first thing that you need to consider is your purpose for holding the seminar. Are you trying to make money on ticket sales? Or money after the class? Are you just trying to get people in the door? Your goal of the seminar will play a huge role in the marketing method that you choose. Dont be afraid to think outside of the box here. Many companies will try to make the most money possible on all fronts. This greedy train of thought will leave your seminar lacking some very important elements. Choose one and stick with it. Many seminar gurus know that the real money in the seminar business is in the after class sales. This could be follow-up work from your instructor, a subsequent training seminar, an add-on product, etc. You want everyone to leave with something in their hand.

Now that you have your goal set, its time to put butts in chairs! Dont let anyone fool you into thinking that a successful seminar has to cost a lot of money. If your overall goal is to get after the class sales, or fill up a room consider some of these options:

Give it away! Its a radical idea, but one that I have grown to know and love. By offering the class for free, you eliminate many of the barriers to the sale. Price is not an issue, and they will expect less from you. That doesnt mean that you should skimp on content, but their expectations will be considerably lower. If you have the resources and time, consider making a pre-seminar call to touch base with everyone that is signed up. This will encourage the students to show up on time, and help built a relationship for future sales.

Submit a press release. Create a simple press release (1 page or less) highlighting your company and your seminar. Dont forget to include your registration phone number. Submit that release to your local paper, and wait for the phone to ring. If the paper decides to pick it up- you get free publicity!

Cross-marketing opportunities. Depending on the seminar, you may be able to find cross marketing opportunities. For instance, if you were doing a financial based seminar teaching students how to file their taxes, you could pair up with a local tax preparation office or accountant. They could pick up half of the marketing tab, or advertise to their existing client base. The possibilities are endless. I have had some great results by pairing with the local chamber of commerce or bank chains. They will offer their seminar space for free, and help with free advertising.

Local paper. Get a price quote from your local paper. Small town publications will usually cost less than $500 dollars, while metropolitan areas can run as much as $2000.

Mailers. Direct mail is always a popular choice, but tends to cost a bit more to execute. Consider forgoing the expensive four color brochure in place of a targeted letter, hand signed. They cost a lot less to reproduce and tend to draw more attention than a mass mailer. Postcards are also an inexpensive way to reach a large group of people. Consider sending your mailers to pre-existing customers or you can buy a targeted mailing list from companies like www.infousa.com.

No matter what method you choose, try to get your campaign rolling 2-3 weeks prior to the seminar. The marketing pieces should be in their hands within that time, anything before or after will likely go unnoticed.

Now that you have attendees, you need to focus on the seminar itself. There are many barriers to adult learning, but they are easy to overcome. The main one that I have come across has to do with the format of the class. You want to create an environment that will encourage teamwork and participation. The instructor plays a huge role in the overall feel of the seminar. They need to guide the students to the right answers, allowing them to try to find the answers in their workbooks or among each other. When creating a workbook, keep the sentences short and to the point. Avoid grammatical errors and long drawn out paragraphs. When possible, use bullet points to break up large amounts of data. If the subject warrants it, add plenty of graphics or screenshots. Over 65% of the general population is visual learners, so make sure to give them plenty to see.

Last but not least, focus on the seminar itself. Dont get caught up in catered lunches or complex setups. Offer tea and coffee and plenty of breaks throughout the day. They are there to learn and hopefully give you their money, the rest is just details!

Rare 1856 Double Eagle gold coin sells for US$345,000 in California

An exceptionally rare coin, which had been hidden in a family collection for over a century, sold at Heritage Auction Galleries Californian Coin Auction for US$345,000.

The 1856-O Double Eagle gold coin, rated as XF45+ (XF meaning Extremely Fine) was discovered in July 2010, having been held by a family after it had been held in the James Bullock collection. Given a value in the auctioneer’s catalogue of $220,000, it quickly rose past that with the help of the 5,000 bidders Heritage say were involved in the auction.

The coin is believed to be one of 20 or fewer examples available commercially. Two are in the Smithsonian Museum.

The President of Heritage Auctions, Greg Rohan, said, “We were all quite impressed overall with how these coins performed”. He continued, “Collectors continue to respond enthusiastically to the best and rarest examples, as evidenced by the heated competition for the Bullock 1856-O double eagle. We don’t expect to see a drop-off in gold demand as the year comes to a close and we hold our last few auctions of 2010.”

Overall, Heritage said that a $13.4 million total was achieved with 96% of lots sold.

Small aircraft crashes into building in New York City

A small aircraft struck an apartment building in New York this afternoon, killing the pilot and a flight instructor. Cory Lidle, a pitcher for the New York Yankees baseball team, was the registered owner of the aircraft and is believed to have been piloting it; his passport was found on the scene. The flight instructor was Tyler Stanger.

The plane, a Cirrus SR20 with registration number N929CD, hit the 26th floor of the Belaire Condominium, a 50 story brick luxury residential building on the Upper East Side at 524 East 72nd Street at York Avenue near the East River in Manhattan, New York City, of which the first 20 floors are a hospital. An eyewitness, present half a block from the building, reported that the plane hit the building, creating an enormous fireball, broke in two and crashed down onto on the street below. Authorities received a 911 call reporting a crash at 2:42 p.m. Eastern time.

New York City mayor Michael Bloomberg gave a press conference at approximately 5:20 p.m. Eastern time to report that the plane was occupied by a flight instructor and a student pilot, but as next-of-kin had not been able to be notified, the identities of the two people on the plane would not be released at this time.

Apartments were seen to be engulfed in flames. The FBI has stated that it was not an intentional attack. The New York area was grey and overcast during the time at crash; however, visibility was not hampered.

After an hour and a half, the fire was extinguished by the FDNY. According to CNN Television ten people have been injured, six of them firefighters. The New York Times, however, reports that eleven firefighters have been injured.

Initial reports suggested that a helicopter was involved, but the FAA has stated that it was a fixed-wing aircraft. CNN Television reported that it was a single-engine fixed-wing plane which left Teterboro Airport, a busy General Aviation airport in New Jersey, circled the Statue of Liberty was tracked on radar until it was lost near the 59th Street Bridge, that the plane may have been having fuel problems or fuel pump problems, and that it was “a pilot in distress.”

Wallace Sines, a source for CNN stated he believes the plane was a Cirrus SR-20 with an installed parachute, which did not deploy. The whole-plane parachute system may have saved the lives of the aircraft occupants had it been safely deployed clear of buildings, but the system is not designed to prevent the trauma associated with a plane impacting a builing. The Cirrus SR-20 was introduced in 2001 and the Cirrus line of 4-seater aircraft has since become one of the most purchased single engine aircraft in the world. The SR-20 does not normally carry an airline-style flight-data recorder, but some are equipped with GPS equipment which logs flight direction, speed and altitude.

The aircraft was owned by baseball player Cory Lidle. He was on board reported by AP. Lidle was killed, according to reports. CNN Television reported that the FBI stated he was at the controls as the only occupant of the plane, and that his passport was found on the ground below the accident.

A little over an hour and a half after the crash, the fire was extinguished after 39 fire units and over 100 fire fighters responded.

The White House has said that there has been no change in the terror threat alert level and that President Bush is being updated constantly.

La Guardia airport was temporarily restricted to no take offs from other airports, however by 4:10 Eastern Time, CNN Television reported that all New York-area airports were open.

CNN Television announced at 3:50 p.m. Eastern Time that as of a few minutes earlier, NORAD is putting fighter aircraft on patrol over certain major American cities as a precautionary measure similar to the actions taken after the 9/11 attacks as a “just in case” measure. It also reported that tomorrow is the 6th anniversary of the attack on the USS Cole in Aden, Yemen.

Moments after the crash, there was a drop in the New York Stock Exchange, however it quickly returned to normal after it was reported that the crash was an accident.

The New York Yankees organization confirmed the plane is registered to Cory Lidle of the New York Yankees, who was planning to fly from New York to Florida, and that some unnamed member of the Yankees organization was on the plane.

One eye-witness interviewed by the BBC stated: “I was wondering why the plane was doing acrobatics and then the next thing I knew was that it had crashed into the building.”She also added that the plane was a small, white, 4-seater winged aircraft and not a helicopter as many news agencies were reporting it to be.

CNN Television broadcast reports from eyewitnesses who reported:

  • A pilot who saw the impact stated, “It looked like a pilot who was desperately trying to get to an airport.”
  • Another eyewitness who saw the event from the Brooklyn-Queens Expressway, also referred to the incident as appearing as if the plane was “desperately trying to get to LaGuardia” and that as to whether he “clipped” the building, or struck it directly, that “he hit it dead on.”
  • “I heard a buzzing noise and then an explosion which looked like a mushroom cloud.”

Aerosmith sued over late cancellation of gig in Maui, Hawaii

Fans of the American hard rock band Aerosmith have launched legal action against the band in response to a late cancellation of a scheduled concert on the Hawaiian Island of Maui.

Attorney Brandee Faria filed a class action suit in Hawaii Circuit Court on October 19. The suit alleges that the band’s cancellation cost fans between US$500,000 (€349,944) and $3 million (€2.1 million) in travel and accommodation costs, as well as other related expenses.

The sold-out September 26 Maui concert – originally planned months before as the final show of a world tour that began in Brazil in April – was canceled by the band on the basis that they could not make it to the island in time after a September 24 concert in Chicago. The Chicago concert, which attracted 18,000 people, was rescheduled at the last minute after the original September 10 concert date had been postponed due to illness.

The band canceled the show at Maui’s War Memorial Stadium, which was set to be attended by 9,000 people, and apologized to fans. The band’s management company, HK Management Inc., gave no initial reason when they canceled on September 20, but blamed logistical reasons by the next day.

However, just days later, on September 29, the band traveled to the neighbor island of Oahu to play a corporate event in Honolulu for Toyota car dealers and private guests. An audience of 6,000 people celebrated the 50th anniversary of the Japanese automobile manufacturer at the University of Hawaii, with Toyota paying $500,000 (€349,944) to hire the campus. Aerosmith received $1 million (€700,000) to perform at the event. Faria alleges that Aerosmith abandoned the scheduled public concert on Maui in favor of the more lucrative corporate event on Oahu.

“…Defendants simply canceled the only public performance by Aerosmith in favor of the larger Chicago venue and the lucrative, private concert for the Toyota car dealers,” the complaint states.

Local officials had hoped the concert would attract other big names to the island.

Faria said that “I’ve had people contact me being out of pocket at much as $800 or $900,” adding that if the cancellation is found to be deceptive ticket holders may be eligible for a minimum of $1,000 each. The complaint also says that those aged 62 or over should receive at least $5,000 each. Currently about a dozen ticket holders are involved with the suit. If the action is approved by a judge, steps will be taken to contact everyone who purchased a ticket.

Bolivian troops told to seize natural gas fields

Bolivian President Evo Morales has ordered that all foreign-owned natural gas fields be turned over to the national government of Bolivia.

President Morales signed a decree that orders troops to seize the fields “immediately” to ensure gas production. The decree also says that companies have 180 days to sign over their fields or leave the country.

The fields are owned by such companies as the United States‘ Exxon-Mobil Corporation, Brazil‘s Petroleo Brasileiro SA, Spanish-Argentine Repsol YPF SA, and Great Britain‘s BG Group PLC and BP PLC.

“The looting by the foreign companies has ended. We are not a government of mere promises, we follow through on what we propose and what the people demand. We want to ask (the Armed Forces) that starting now, they occupy all the energy fields in Bolivia along with battalions of engineers,” said Mr Morales after signing the decree.

“The time has come, the awaited day, a historic day in which Bolivia retakes absolute control of our natural resources,” added President Morales.

One of Morales’ vows in his presidential campaign was to “recover” the country’s natural resources by renationalizing them. President Morales explained, on a visit to Brazil in January, that renationalising the industry would not mean expelling foreign companies or expropriating foreign property. “Foreign companies have every right to recover investments and make profits, but profits should be balanced”.

Bolivia has the second largest supply of natural gas in South America after Venezuela.

Community Television consortium urges Australian government to increase funding for channels

The Community Broadcasting Association of Australia (CBAA) has urged the Australian government to increase funding for community broadcasters. CBAA is asking for an extra $AU14 million for “vital community broadcasting services.”

According to CBAA President Deborah Welch, community television is the training ground for the Australian media industry.

“It is the launching pad for the career of thousands of Australian musicians. It is an incredible source of local news, music and culture targeted specifically to local communities and produced by members of the community themselves,” Ms Welch said in a statement.

Stations broadcast a range of programs from religious programing, to car shows and sporting events. Several state based sporting leagues broadcast on community television stations. C31 Melbourne broadcasts an association football show called The Victoria Football Show; Queensland Community Television broadcasts AFL Queensland State League matches; and C31 Adelaide broadcasts Indoor Cricket & Netball and South Australian National Football League matches.

According to the CBAA, the primary income sources for community television stations are sponsorship announcements and community donations.

“The CBBA is calling on the Federal Government to commit $14 million in new funding for content production, infrastructure, training and sector co-ordination and planning,” Ms Walch says. “With $10.4 billion being spent on ‘strengthening the economy’ this is highly targeted $14 million will assist in skills development and employment pathways for many volunteers involved in local stations as well as strengthening local communities ability to sustain themselves in tough times.”

Australian Greens communications spokesperson Senator Scott Ludlum says, “[The] government should always be looking at ways to bring communities together, through sharing information and building partnerships.”

Chrysler files for bankruptcy, Fiat Group SpA to run company

The American auto company Chrysler filed for bankruptcy Thursday, however a deal with European auto maker Fiat went through. The emerging Chrysler will be owned 55 percent by the United Auto Workers, eight percent by the United States Government, two percent by the Canadian Government and Fiat would begin with a 20 percent share.

Chrysler Chief Executive Robert Nardelli will step down when the bankruptcy proceedings are finalized.

Initially, the Italian company, Fiat, will appoint three members on the emerging Chrysler’s new board, and the United States government will appoint six. Fiat can assume the majority of the ownership upon repayment of American loans.

Chrysler administrators expect that the bankruptcy should take a couple of months.

“We expect this to be a very short, 30-to-60-day bankruptcy process, during which the company will function normally,” a top administration official said, “People will be able to buy cars, they will have their warranties honored, and everything should go on normally.”

The bankruptcy filing indicated that Chrysler was in debt to 20 creditors to a tune of more than $10 million each.

Meanwhile, the deal with Fiat did go through, and Chrysler should have cars designed by Fiat out on the market by 2011.

“It’s a partnership that will give Chrysler a chance not only to survive, but to thrive in a global auto industry,” said American president Obama, “Fiat has demonstrated that it can build the clean, fuel-efficient cars that are the future of the industry.”

However, automotive analyst Erich Merkle has hesitations.

“History would show that alliances really don’t work that well,” Merkle said, “even though, no matter how good they may look on paper.”

The restructuring has been managed by Steve Rattner, a former investment banker, and the U.S. Government auto task force.

Obama has set three ultimatums before Fiat. Fiat should produce a 40 mile per gallon vehicle while managing the new Chrysler, transfer fuel efficient Fiat technology to Chrysler factories in the United States, and produce cars in Chrysler factories and distribute them in Europe.
