Study: cognitive therapy as good as medication, but lasts longer

Wednesday, April 6, 2005A landmark study comparing effectiveness in reducing major depression of two forms of initial treatment — cognitive therapy which involves discussing thought processes with a qualified counselor, vs medication — has revealed that both forms of therapy reduce depression, but that the benefits of cognitive therapy last longer than those of medication. The study finding contradicts current treatment guidelines of the American Psychiatric Association (APA).

The 16-week study, by researchers at the University of Pennsylvania and Vanderbilt University, was the largest trial yet undertaken to resolve the years-old debate. Two-hundred-and-forty patients with moderate-to-severe depression were given one or other of the treatments, or a purposely-ineffective placebo. Study representatives described the two treatments about equally effective — both much more effective than the placebo — but with the positive effect of talk-therapy lasting longer.

In fact, the cognitive therapy group saw only as many relapses as a another group that had continued to receive drug therapy.

“We believe that cognitive therapy might have more lasting effects because it equips patients with the tools they need to learn how to manage their problems and emotions,” said Robert DeRubeis, professor and chair of Penn’s Department of Psychology. “Pharmaceuticals, while effective, offer no long term cure for the symptoms of depression. For many people, cognitive therapy might prove to be the preferred form of treatment.”

“Medication is often an appropriate treatment, but drugs have drawbacks, such as side effects or a diminished efficacy over time,” DeRubeis said. “Patients with depression are often overwhelmed by other factors in their life that pills simply cannot solve. In many cases, cognitive therapy succeeds because it teaches the skills that help people cope.”

Researchers pointed out that while the effectiveness of cognitive therapy depended largely on the experience of the practitioner, the same was true of medication, due to the judgement required in prescribing correct dosages of drugs.

Due to the damage by Hurricane Katrina and subsequent flooding, a number of colleges and universities in the New Orleans metropolitan area will not be able to hold classes for the fall 2005 semester. It is estimated that 75,000 to 100,000 students have been displaced. [1]. In response, institutions across the United States and Canada are offering late registration for displaced students so that their academic progress is not unduly delayed. Some are offering free or reduced admission to displaced students. At some universities, especially state universities, this offer is limited to residents of the area.

Mumbai officials demolish 39K shanties; 200K homeless

Officials in Mumbai, India, demolished over 6,000 shanties today in a push to eradicate the capital city’s slums. In total, 39,000 shanties have been flattened, displacing over 200,000 people, in the city’s biggest-ever demolition drive, which began in early December.

When complete, over 2 million people are expected to be displaced. After wiping out the least desirable shanties, next in line for demolition are the illegal ‘well-off’ shanties and neighborhoods, according to the legal and bureaucratic motions that have been executed toward cleaning up Mumbai’s appearance by lowering the dominance of shanties, which make up 62 percent of Mumbai’s housing.

“As far as eye can see, there are mounds of wood, tin and tarpaulin, the remains of 6,200 illegal homes, flattened by a heavy excavator running on tank-like tracks and giant motorised claws,” the Indian Express reported about today’s destruction. [1]

Chief Minister Vilasrao Deshmukh said that citizens would see a change within six months. “Every chief minister likes to be remembered, and I’m no exception,” said Deshmukh, who despite having an empty exchequer, also announced that Rs 31,000 crore will be spent on new roads, sea links and rail lines. High percentage of US patients on placebos without knowing it

A new study amongst doctors in the United States on the use of placebos—pills with no medical effect—shows that almost half of the questioned practitioners prescribe placebos, most of them within the last year.

The majority of 466 faculty physicians at Chicago-area medical schools interviewed by a research group of the University of Chicago stated that placebos are useful to calm a patient down or to respond to demands for medication that the doctor disagrees with, i.e. “to get the patient to stop complaining”.

96 percent of the physicians surveyed believe that placebos can have therapeutic effects. Close to 40 percent stated that placebos could benefit patents physiologically as well as mentally.

Twelve percent of surveyed physicians think that placebos should be banned from clinical practice. Among the doctors who prescribed them, one in five said they outright lied to patients by claiming a placebo was medication. But more often the physicians came up with ways to explain like that “this may help you but I’m not sure how it works.”

The American Medical Association (AMA), the largest association of U.S. doctors and medical students, tells its members that “[p]hysicians may use placebos for diagnosis or treatment only if the patient is informed of and agrees to its use.” The research, published in Journal of General Internal Medicine this week, is the first major U.S. study of doctors on the use of placebos since 1979.

Kansas library discusses Wikipedia

A diverse group of approximately 25 people gathered at the Johnson County Library on Monday to participate in reference librarian Scott Vieira’s class, Wikiwhatia? Wikipedia.

Scott opened the session by sharing a disparaging quote from Robert McHenry (former editor-in-chief of Encyclopedia Britannica) comparing Wikipedia to a public restroom. He then shared a quote from Andrew Keen, author of The Cult of the Amateur, in which he stated Wikipedia is the blind leading the blind. Scott shared Wikipedia usage data from a 2007 Pew Internet study showing 36% of adults have consulted Wikipedia and that that Wikipedia receives 10,000-30,000 searches per second. Scott also pointed out that Wikipedia now has over two million articles in English alone (over nine million articles in 250 languages). So there is a discrepancy here, lots of critics and lots of use.

Scott then shared some historical information in order to provide a context for understanding Wikipedia. This included some important names and dates in the history of encyclopedias… including Pliny the Elder (23-79 C.E.) who published 37 Volumes of Natural History, and Joachim Sterck van Ringelbergh (c. 1499-1556) who first used the term encyclopedia, and then d’Alembert and Diderot who published 17 volumes of their French Encyclopedia from 1751-1765. Encyclopedia Britannica was first published in Scotland in serial format 1768-1771.

Scott also discussed more recent history, sharing a photo of Ward Cunningham –who is credited with being the inventor of wiki software. Wikipedia was founded on Jan 15, 2001 by Larry Sanger and Jimmy Wales. Sanger left Wikipedia in 2002 and founded Citizendium in 2007 a complementary project which now has 4500 articles.

After the historical perspective, the class moved into using Wikipedia. Everyone in the audience had used Wikipedia. The class explored the content of Wikipedia, realising there is a whole lot going on. Scott demonstrated that the article about “frogs” for example, is semi-protected. The “history of science” article was examined, including the history of changes. Discussion covered how people contribute, who contributes and edits. The group also talked about Wikipedia bots, which aid in routine tasks in a semi-automated or automated fashion.

A discussion about teachers, school media specialists and students regarding their use of Wikipedia ensued. Some teachers and school media specialists are negative about Wikipedia, but Scott’s hope is that they will use it to start a discussion about the need to evaluate and critically think about information (even when it’s from more traditionally reviewed and edited sources).

An unanswered question from an audience member was, “What was the first Wikipedia article?”

Discussion topics from Scott to audience were as follows:

  • How do we determine authority on a subject? How important is it that an article be written by an expert? How is Wikipedia changing our idea of what an authority is?

1. Mail Order Marketing – At some point pretty much all of us have open the mailbox to find a catalog from a company such as Victoria Secrets or REI or one of a thousand other companies that use mail order marketing. Smaller companies might send out something smaller such as a flyer or brochure, but the intent is the same. They are hoping you will be excited about the products they are offering and that the catalog or flyer will entice you to buy something.

This type of marketing can be successful if you happen to get the right person at the right time. The best way to do that is to send it to pretty much everyone or at least find a “list” that groups people according to their interests so you only send your information to people who might use it. The success rate usually is less than 5% so you will need to send out a lot of information in order to be successful.

This is a very hard way for the average person to start a business. Especially if you consider this is just the marketing part of the business, you still have to worry about getting a product, making the catalog, taking orders and finally shipping. A lot of work and a lot of initial expense.

2. Conventional Marketing – Everywhere you go you are bombard with advertisements trying to show you how wonderful some product is and how you can’t live without it. This is the primary marketing plan of many traditional businesses. Coke, Pepsi, Nike and many others spend millions of dollars to get you to purchase their brand.

As a small business you would also be trying to get people to use your product and service. Local radio and TV stations, billboards and newspaper ads are all good ways to get your name out to the public. As with any business and advertising, eventually a word or mouth campaign can also help with your business promotion.

Again this is a money intensive way to get the word out. You still have all the other problems of having a store front or place people can go to get your product or service. You quite often will have employees and all the work and expense that go along with them. Once again, this type of business takes a lot of money to get started.

3. Direct Marketing – Remember the door to door salesman? It was something done in the past much more than today but most of us can remember the Kirby vacuum salesman or maybe the Fuller brush man. Today, there are still those direct sales companies but they do less of the door to door sales and have converted over to things like “home parties” and other venues like home and trade shows. Examples of this type of business are Mary Kay, Tupperware and even Avon.

The advantage of this marketing system to a small business or home business person is there is no need to have a storefront and the expense that goes with it. There is also no product to produce; the parent company has already taken care of that. It also does a lot of the work as far as brand recognition and warehousing and shipping of your product. You might be required to keep a certain amount of inventory on hand but it can quite often be kept in a closet or spare room within your house.

With less initial startup cost, very little advertising cost (you and word of mouth are your advertising), and no employee or storefront expense this is a much better option for the average person looking to start a home based business.

4. MultiLevel Marketing – MLM and direct marketing are similar in many respects. An MLM company uses its money to motivate its distributors and not towards the national public through advertising campaigns. When looking into MLM, you may have heard of companies such as Shaklee or Scent-sations. The biggest difference between Direct Marketing and MLM is that Multilevel Marketing is concerned with selling product but adds the team building aspect.

You, as a network marketer, would help others create their own home based business and give them training on how to build it stronger and in turn help even more people. This is called your downline. You get paid not only on what product you sell but the product they sell as well. Therefore it is in your best interest to help your downline become successful, just as it is important for your upline to help you be successful.

The parent company uses the money it saves from advertising to pay you. They also spend it on improving the product, creating more products for you to promote, creating new tools for you to be successful, giving education classes, and offering other bonuses such as money, trips, cars and other incentives.

They take care of all the production costs, the warehousing, the handling of payments and the shipping. Most of the good MLM companies don’t require you to keep inventory on hand but they do require that you purchase and use their product. Each company has a different requirement on how much you are required to purchase each month in order to be a distributor.

Like direct marketing this plan is a great starting place for a majority of people looking to start a home business. It has very little startup cost and very little risk. You can work this business while keeping your “real job”. It can be used as a way to make just a little extra each month or if it is your goal, you can use it to replace your current income.

Whatever you choose make sure it fits your goals and aspirations. It is good to go through and create a vision map of what you are trying to accomplish and where you see you and your business in the future.

Find a mentor, friend or coach who has been there and discuss the options of whatever plan you decide on. Starting a home business or small business is a big decision and one that you should research before just jumping in with both feet.

Kennedy Center names 2007 honors recipients

The Kennedy Center announced that its 30th presentation of the Kennedy Center Honors would go to pianist Leon Fleisher, comedian Steve Martin, singer Diana Ross, director Martin Scorsese and musician Brian Wilson. The Center was opened to the public in 1971 and was envisioned as part of the National Cultural Center Act, which mandated that the independent, privately-funded institution would present a wide variety of both classical and contemporary performances, commission the creation of new artistic works, and undertake a variety of educational missions to increase awareness of the arts.

In a statement, Kennedy Center Chairman Stephen A. Schwarzman said that “with their extraordinary talent, creativity and perseverance, the five 2007 honorees have transformed the way we, as Americans, see, hear and feel the performing arts.”

Fleisher, 79, a member of the Peabody Institute‘s music faculty, is a pianist who lost use of his right hand in 1965 due to a neurological condition. He became an accomplished musician and conductor through the use of his left hand. At 67, he regained the use of his right hand. With the advent of Botox therapy, he was once more able to undertake two-hand performances in 2004, his first in four decades. “I’m very gratified by the fact that it’s an apolitical honor,” Fleisher said. “It is given by colleagues and professional people who are aware of what [an artist] has done, so it really is apolitical — and that much more of an honor.”

Martin, 62, a comedian who has written books and essays in addition to his acting and stand-up comedy career, rose to fame during his work on the American television program Saturday Night Live in the 1970’s. Schwarzman praised his work as that of a “renaissance comic whose talents wipe out the boundaries between artistic disciplines.” Martin responded to the honor saying, “I am grateful to the Kennedy Center for finally alleviating in me years of covetousness and trophy envy.”

Ross, 63, was a product of Detroit‘s Brewster-Douglass Projects when as a teeager she and friends Mary Wilson and Florence Ballardis formed The Supremes, a ground-breaking Motown act. She portrayed singer Billie Holiday in the 1972 film Lady Sings the Blues, which earned her an Oscar nomination and a Golden Globe award. “Diana Ross’ singular, instantly recognizable voice has spread romance and joy throughout the world,” said Schwarzman. Ross said she was “taken aback. It is a huge, huge honor and I am excited to be in this class of people.”

Scorsese, 64, is one of the most accomplished directors the United States ever produced, whose work includes Mean Streets, Taxi Driver, GoodFellas, Cape Fear, The Last Temptation of Christ and The Departed, for which he won a 2006 Academy Award for Best Director after being nominated eight times. Scorsese said, “I’m very honored to be receiving this recognition from the Kennedy Center and proud to be joining the company of the very distinguished individuals who have received this honor in years past.”

Wilson, 65, along with his brothers Dennis and Carl, formed the Beach Boys in 1961. They had a series of hits that included “Surfin’ U.S.A.” and “Wouldn’t It Be Nice.” Their 1966 album Pet Sounds is considered one of the most influential recordings in American music. “This is something so unexpected and I feel extremely fortunate to be in the company of such great artists,” said Wilson, who is currently on tour.

The Kennedy Center’s board of trustees is responsible for selecting honorees for “lifetime contributions to American culture through the performing arts.” Previous honorees, including Elton John and Steven Spielberg, also submitted recommendations. A wide variety of people were under consideration, including Emanuel Ax, Evgeny Kissin, Renee Fleming, Laurence Fishburne, Francis Ford Coppola, Melissa Etheridge and Kenny Chesney.

President Bush and first lady Laura Bush will attend the center’s presentation at its opera house on December 2, 2007, which will broadcast on December 26 on CBS.

Annual rich list shows Ireland now has six billionaires

Sunday, April 3, 2005The annual Sunday Times Rich List published today showed that Ireland now has six euro billionaires, up from four last year. The combined wealth of Irelands 250 wealthiest is €42.3bn.

Topping the list was the new entrant Hilary Weston with a fortune estimated at over €7.2bn. Weston was excluded in previous lists because the majority of her wealth was made abroad, however a rule change this year allowed the Dublin born model to enter in first place. Mrs Weston, 63, grew up in Ireland and built her modeling career there before becoming wed to Galen Weston, heir to his grandfathers multi-billion euro retailing fortune in Canada. The couple permanently moved to Canada in 1983 after a kidnap attempt by the IRA.

The top six on the list were as follows:

  2. Sir Tony O’Reilly; €1.897bn – Food, media and inheritance
  3. John Dorrance; €1.857bn – Inheritance
